Your First Visit
Your initial physical therapy evaluation at our River North Chicago clinic is a 60-minute, one-on-one assessment with a licensed physical therapist. We collect objective measures of muscle strength, range of motion, joint mobility, posture, movement patterns, and functional capacity, then use them to build an evidence-based, individualized plan of care and a customized home exercise program.

You'll work with the same physical therapist from your evaluation through discharge.
What to Bring
- A photo ID and your insurance card
- Any imaging reports, surgical notes, or physician referral, if you have them
- A list of your current medications
- Comfortable clothing that lets us see and move the area being treated

Insurance and Payment
We're in-network with Blue Cross Blue Shield and will verify your benefits before your first visit. For most out-of-network plans, such as Cigna, Aetna, and United Healthcare, we submit the claim for you. We also offer self-pay rates and packages. Learn more about insurance and self-pay.
